Insurance
Depending on your current health insurance provider or employee benefit plan, it is possible for services to be covered in full or in part. Please contact your provider to verify how your plan compensates you for mental health/psychotherapy services.
We recommend asking your insurance provider the following questions to help determine your benefits:
- Does my health insurance plan include mental health benefits?
- Do I have a deductible? If so, what is it and have I met it yet?
- HopeAllianz is considered a specialty clinic (SP). Will I have a copay? If so, what is it?
- Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?
- Do I need written approval from my primary care physician in order for services to be covered?
- Is HopeAllianz Inc (EIN 26-0069922) covered in my network?
Payment Options
HopeAllianz accepts cash, check and credit cards for InPerson and OnLine services. Health Savings accounts and Spending Accounts may also be used to pay for InPerson and OnLine services. For OnLine sessions a credit card will need to be on file with HopeAllianz to charge you – following your session.
HopeAllianz is contracted with the following insurance networks and may be used for InPerson sessions only: Aetna, BCBS, HealthPartners, Magellan, Medica, Medicaid, Medicare, MultiPlan, TRICARE, TRIWEST, UCare, UnitedHealthcare UHC/UBH.
For Out-of-Network benefits check with your insurance provider to determine what services may be covered, the fee must be paid at the time of your session. We will provide you with an invoice you can then submit to your insurance company for reimbursement of what they will cover.
A sliding fee is available for clients who will not be utilizing their Out-of-Network benefits, HSA or FSA benefits and who are unable to afford the full fee.
Cancellation Policy
HopeAllianz requires a 24 hour cancellation notice for any scheduled appointments. Failure to attend a scheduled session without giving 24-hour notice will be subject to a full fee charge. We have reserved this time for you and it is held exclusively for you. In addition, if you are late for a session, you may lose some of that session time.
